The Role and Department:
We have an opportunity for a proactive Care Coordinator to join the organisation on a fixed term basis. This is an opportunity to work as part of a charity committed to our vision of enabling everyone in our communities to achieve better mental health. TOG Mind is a fast-paced charity delivering a wide variety of mental health and emotional wellbeing services. In your role as a CYP Population Health Care Coordinator you will be part of an innovative population health management programme focused on providing more proactive identification and support to children and young people (aged 5-17) at risk of developing mental health condition. You will work across the West neighbourhood from community settings, using a data led approach to identify a cohort of young people to be supported through the offer. You will be responsible for making initial contact, coordinating appointments and being a first point of contact to regularly review care planning. You will work as part of the PHM team and be supported by Tameside Oldham and Glossop Minds dedicated children, young people and family’s directorate and you will also connect with our Adults Navigator who supports the West district
What you will bring to Mind:
To be considered for this role, we are looking for someone who is passionate and driven to work in a data driven role to provide early mental health and wellbeing support to children and young people.
You will have experience in holding a caseload of children and young people and you will be able to review the progress of holistic care plans to ensure young peoples needs are being met at the earliest opportunity. You will bring strong interpersonal skills with the ability to build trusting relationships, foster collaboration and communicate empathically with families.
You will bring knowledge and understanding of the Health and Social Care sector and have a tenacious approach to cross sector working, ensuring a holistic approach is taken to address education, health and social needs. You will be able to demonstrate active listening and curiosity skills and value preventative strategies in supporting mental health and wellbeing.
In addition, you will bring a flexible approach, committed to delivering the programme as effectively as possible, being accepting of change and seeking insight, learning and growth through the process.
A willingness to undertake training and qualifications relevant to the role is also important. We value individuals who are committed to personal and professional growth and who can contribute positively to our team and the communities we serve.
