Job Description
Creative Living Centre (CLC) is a registered charity, offering a person-centred, holistic approach to supporting people experiencing poor mental health and wellbeing in Bury (Greater Manchester).
JOB DESCRIPTION
As a member of the Creative Living Centre (CLC) Service Delivery Team, the Mental Health and Wellbeing Support Co-ordinator is responsible for ensuring that new and existing members (service users) are provided with both initial and ongoing emotional support in either 1:1 or group sessions, as appropriate to their needs which are identified and reviewed on an ongoing basis.
Role Responsibilities
Overview
- Complete 1:1 appointments with new / returning members, working in a person centred, holistic way to understand their needs and to recommend appropriate support sessions and/or therapies.
- Help people to choose initial activities from the centre timetable and put into place initial goals relevant to their needs.
- Provide regular 1:1 emotional & practical support where appropriate for members to support their mental health and wellbeing.
- Deliver workshops, sessions, and courses to groups both internally and externally. This may be psychoeducational, physical and/or creative.
Service Delivery
- Complete relevant paperwork and forms accurately, taking into consideration confidentiality and GDPR.
- Maintain clear, timely and accurate records of all work including class attendance.
- Liaise with other services / professionals on behalf of members where appropriate.
- Refer / signpost members to other services depending on their needs.
- Discuss more complex cases with staff team and escalate support to the service delivery leads/ DSL(s) where appropriate.
- To ascertain whether someone is at risk (e.g., of abuse, self-harm or suicide) and take initial action according to the Centre’s policies, including involving or notifying the DSL team, supporting referrals, and maintaining accurate records.
- Represent the CLC at external events, e.g., to staff a CLC information stall.
Administration
- Ensure all required paperwork is filled in and stored in the appropriate place.
- Input work records to the CLC database.
- Update member attendance to appointment records, courses and sessions on database.
- Support with data analysis where needed.
- Complete case studies where appropriate in support of work completed with the CLC.
- To update and maintain course and workshop materials, including developing new materials where required.
General
- Be aware of and comply with the Centre's policies of Confidentiality, Health and Safety, Fire Safety, Safeguarding (and all other relevant policies), and to follow the staff code of conduct.
- Attend staff, centre and other meetings as required.
- Participate in activities and events that support mental health awareness and advocacy.
- Take part in supervision sessions and ongoing training and development opportunities as part of Continued Professional Development.
- Take part in other duties which are felt to be relevant to the role and which may not be specified above.
